  2. PARTICIPANT WAIVER OF LIABILITY AND HOLD HARMLESS AGREEMENT  

  4. In consideration of the undersigned being given the opportunity to take part in a Village Event in any manner whatsoever, presented, sponsored, operated or provided by Village of Flossmoor, the undersigned hereby recognizes, acknowledges and assumes any and all risk of injury (including death) pertaining to observing or taking part in a Village Event.

    The undersigned also acknowledges that they have received and reviewed the performance contract relating to the performance on September 27, 2022 and agrees to participate voluntarily and at their own risk.  The undersigned affirmatively states that they do not suffer from any type of ailment, illness or disorder that affects or may affect the undersigned’s ability to participate in as a performer at the event.  The undersigned hereby acknowledges and assumes any and all risk of injury (including death) with relation to any type of ailment, illness, disorder or abnormality, whether known or unknown.

    The undersigned hereby releases and agrees to defend, indemnify and Village of Flossmoor, its officials, agents and employees harmless from any and all liabilities, claims, injuries or damages of whatever nature, arising from, as a result on account of, directly or indirectly, observing or participating in the event.

    Any and all questions or concerns shall be brought to the attention of Stephanie Wright prior to participating in the event. The Village of Flossmoor reserves the right to deny participation to any person for any reason whatsoever.

    This Waiver of Liability and Hold Harmless Agreement shall be governed by the laws of the State of Illinois and jurisdiction shall be proper only in the County of Cook, State of

    Illinois.  The invalidity or unenforceability of any of the provisions hereof shall not affect the validity or enforceability of the remainder of this Agreement.

    The undersigned must be present and complete this waiver prior to participation in the the event on September 27, 2022.

    By signing below, the participant has read and understands the waiver as described above.

  8. Electronic Signature Agreement
    By checking the "I agree" box below, you agree and acknowledge that 1) your application will not be signed in the sense of a traditional paper document, 2) by signing in this alternate manner, you authorize your electronic signature to be valid and binding upon you to the same force and effect as a handwritten signature, and 3) you may still be required to provide a traditional signature at a later date.

    If the volunteer is under 18 years of age, a parent or legal guardian must sign this agreement on behalf of the volunteer.
